Work half time (0.5 FTE) every other weekend as the on-call lead nurse for a hospice company in the Portland Metro area.  Perform admissions to the service, carry cell phone and respond to incoming calls 24/7 for the weekend.  Complete visits as needed.  Assist patients and families with EOL/palliative care according to their wishes.  Attend all deaths.  Services take place in patient’s homes and in various nursing facilities, assisted/independent living facilities.

Owner of a Nurse Case Management and Legal Nurse Consulting firm that is headquartered in Portland, OR.  Provide National coverage via comprehensive network of nurses.  Work on a contract basis to perform various nursing functions.  Services include: Nurse Case Management, Workers’ Compensation and Catastrophic experts (including Jones Act & Federal), Legal Nurse Consulting, Expert Witness services, review/consultation for setting up new OH/WC programs, performing assessments, Elder Case Management, patient/family teaching, home safety/risk assessments, accompanying clients to medical appointments as an advocate/case manager, and assistance in recruiting in-home caregivers. Other duties are available as requested and are evaluated as discussed with the client (and family as appropriate).

Worked full-time as a Nurse Case Manager for a national company that focuses on Workers’ Compensation.  Worked for the Portland, OR office and covered cases in both OR and WA.  Partnered with insurance claims examiners and attorneys on their current open files that required case management.  Worked together with injured workers, physicians and rehabilitative teams to ensure injured worker was receiving tailored, appropriate, timely and effective care.  Provided on-site job site reviews, catastrophic case management, long-term assignments and task assignments.  Worked with local jurisdiction as well as federal claims.  Cases varied from orthopedic, psychiatric, amputations, cardiac, respiratory and catastrophic after hours, on-call which involved complex distant cases that required up to and including medically evacuation from islands to U.S. mainland for life threatening issues.  Case management style is aggressive and proactive with follow-up documentation being sent immediately following every attended appointment.

Worked full-time as the Program Director for a national medical transportation company.  Worked on-site with TriMet in downtown Portland to perform face to face physical and cognitive assessments of current paratransit clients and new applicants to assess ability to ride fixed route transit services. Duties included supervision of two full-time and three on-call staff members who worked as Mobility Assessment Evaluators.  Worked in collaboration with Operations, MTM and TriMet LIFT to ensure the most appropriate, timely and effective delivery of determinations of functional and cognitive abilities of clients in regards to compliance with TriMet’s requirements.  Oversight of the budget for expenditures at the site level.  Program saved over $1M of TriMet funding in the first year of implementation.

Prior Military: (Military Police) 1992-1998- Entered Active Duty Army in September 1992 as Military Police.  Stationed at Ft. McClellan, Alabama for six months for One Station Unit Training.  First duty station was Hanau, Germany. Worked for the Provost Marshall’s Office.  Went from Active Duty to Reserves and then National Guard for a total service time of six years, E-5 (SGT) when honorably discharged. MOSs included 95B (Military Police), 71L (Admin) and 91C (LPN).
