Life Enhancement Case Manager
I coordinated the completion of the development and implementation of the life enhancement program at Healthspring of Alabama. This included promotion to local hospitals and healthspring employees as needed. I performed intensive case management of and for health plan members with life limiting illnesses. This included utilization management skills, advanced care planning and other facets to assist the member as they progressed through their disease process. This helped the member and saved costs over time.

The Clinical Director is responsible for management of the hospice program in matters of overall operation at the local office level. Conducted focused medical review audits on a daily basis.
I began work with this company on August 19, 2002. My average caseload of 16-18 patients. I was also responsible for supervising the home health aids for these patients as well as taking call for all the patients on the team. The average census at this time for my team was approximately 50-75 patients.
In August of 2003 I was promoted to the position of Lead Case Manager. In this position I was responsible for scheduling of home health aides, assisting the Clinical Director, chart auditing, and various other duties.
