The facility has 2 procedure rooms, 2 O.R.’s, 8 RR beds and 5 pre op admitting beds. We perform approximately 1000-1200 cases per month that include laparoscopic surgeries, (Gallbladders, etc.), pacemaker generator replacements, primary pacemaker placement, pain management, plastic and reconstructive, general surgery, podiatry, orthopedic surgery, ENT, gynecologic, Endoscopic, among others. There are 30 FTE’s as well as 10 per diem RN employees. I am responsible for the scrub techs monitoring their techniques, observation of and practicing infection control guidelines, sterile technique, and overall ability to function effectively during a surgical procedure. I am the administrator/director of nursing, in charge of Risk Management/Quality Assurance programs, credentialing, employment, as well as a first assistant, OR circulator, GI nurse, pre op admitting nurse, and PACU nurse.

The facility had 4 O.R.’s, 11 RR beds. We performed approximately 300-400 cases per month that included all types of laparoscopic surgeries, (Gallbladders, lap bands, tubal ligations, laser ablations, etc), pain management, plastic and reconstructive, general surgery, podiatry, all types of orthopedic surgery, ENT, and gynecologic. Hallmark physicians performed approximately 15 to 25 pediatric procedures (children ages 6 months of age to 17 years of age) per week such as tonsillectomies, adenoidectomies, PE tube insertions. There were 13 FTE’s as well as 15 per diem RN employees. I was the administrator as well as a first assistant, Pre-Op admitting nurse, OR circulator, and RR nurse.

There were 2 O.R.’s and 4 RR beds (I was also in charge of the RR). We performed percutaneous discectomies (cervical and lumbar), open back surgery, implants, pain management cases, discograms, cosmetic, ortho, GYN, podiatric, and general surgery cases. I have first assisted on scores of gastric bypasses and lap bandings. I also worked as a registry nurse, Moreno Valley Hospital, Hollywood Community Hospital, Century City Hospital, circulating general surgery, orthopedics, neurosurgery, gynecology, vascular, urology etc. surgeries. At the BI there are 10 FTE’s. At times I also serve as a CRNFA (Certified Registered Nurse First Assistant), a Pre-Op admitting nurse, a circulating nurse and Recovery Room nurse. I also worked as a registry nurse in surgery, ICU’s (Intensive care Units) for cardiac, neurological, post surgical, cardiac step down at Moreno Valley Hospital, Hollywood Community Hospital, circulating general surgery, orthopedics, neurosurgery, gynecology, vascular, urology, podiatry, etc. surgeries..

There were 3 O.R.’s, 1 procedure room, that performed plastic, orthopedic, GI, pain management, gynecology, podiatric, and general surgery cases, 6 RR beds (I was also in charge of the RR) with 22 FTE’s. I assisted with liposuctions, circulated O.R. Cases, closed wounds, was a Pre-Op admitting nurse, a recovery room nurse, and scrubbed as a CRNFA. As a weekend position,. I was a surveyor, an ASC project manager, and performed site visits to check for adherence to California, AAAASF, CMS, etc. compliance. I also worked as a registry nurse, Moreno Valley Hospital, Hollywood Community Hospital, Century City Hospital, circulating general surgery, orthopedics, neurosurgery, gynecology, vascular, urology etc. surgeries.

There were 2 O.R.’s and one procedure room. We performed plastic, orthopedic, general, gynecology, podiatric, and ENT cases. I was also in charge of the RR. I worked as a CRNFA, Certified Registered Nurse First Assistant, circulated cases, did RR nursing, Pre-Op admitting, etc. I personally performed all the various types of laser surgery done at the clinics under the auspices of the Nurse Practice Act of the State of California. I first assisted on the various types of outpatient procedures performed at the clinic (inguinal hernias, orthopedic procedures, anterior/posterior repairs, cholecystectomies, ventral hernias, face lifts, tummy tucks, upper and lower blephs, breast implants, among others), saw patients post operatively, and I was the in-service director for the facility. I was also responsible for all the various documentation to maintain our state certifications. I was also a consultant to a number of surgery centers in Los Angeles and Beverly Hills in regard to operating room matters, policies and procedures, and training personnel for mock JCAHO and CMS surveys.

I gave advice and technique theory as it related to operating room matters.

My company, Vast Enterprises, LLC., Inc., is provider approved by the California Board of Registered Nursing, provider number 11202. I am a clinical assistant for the University of California at Los Angeles (UCLA) Registered Nurse First Assistant (RNFA) program.

I am a legal nurse consultant and have been doing so since 1983. I spend less than 5% of my time doing such. I give expert opinions and testimony on the standard of care in all types of medical malpractice cases for both defendants and plaintiffs. I serve as a case reviewer/consultant and review nurse malpractice cases for the California Board of Registered Nursing on an as needed basis.
